so you've just been diagnosed with osteoporosis the doctor wants you to maybe take medications or at least think about taking it you're a little hesitant because you've heard some horror stories and you want to try to manage it maybe more naturally with supplements or exercise but are the supplements and exercise as effective as taking some of the osteoporotic medication i'll tell you what i tell all my clients hey everyone welcome to the front row edu physical therapist out of bellingham washington at integrative physical therapy so patients will often ask me look at i just got diagnosed with osteoporosis what do you think i should do should i take the medications or not so first of all that's a discussion that you need to have with your doctor with your spouse with your family decide what's best for you there are side effects of the medications i don't want to be that person or have my wife be that person in that one in 1000 that has osteonecrosis of the jaw or a atypical fracture of the femur or esophageal cancer so there are some risks of these medications there's no doubt just like there is risk for any type of medication but the scope of this video is to talk mainly about whether or not the efficacy or how well the drugs performed compared to a regimen of supplements and exercise there was a study done back in 2012 it was called the comb study comb it's an acronym for the combination of micronutrients for bone and what they did was researchers scoured the literature and they looked at different micronutrients that have been proven through separate studies that have been beneficial for bone growth in order for bones to grow well think about it almost like you want to grow something with the right nutrients in the soil so it's a little bit of this a little bit of that you know we all talk about calcium and vitamin d which are critical but there's a whole host of other micronutrients that are also responsible this is the way they designed the study they had two groups one group was taking the biphosphonate the medication for osteoporosis the other group was taking a cocktail if you will of six different supplements and i'll list all the supplements as well as how much in this particular study they were taking down below in the comments and so you don't worry about writing it down but there were six different supplements combined with daily impact exercises so what were the six supplements vitamin d3 2000 ius vitamin k2 100 micrograms a day strontium 600 milligrams a day magnesium 25 milligrams a day dha 250 milligrams a day calcium 1200 milligrams but mainly from dietary sources what i tell my clients is that we have to be careful about over supplementing because calcium for example you can get too much of a good thing so what i recommend to my clients is take a food log for approximately three days to get an average intake of calcium per day most people end up eating the same thing so i usually have a cup of yogurt in the morning and then i usually have a little bit of cheese here and a little bit of milk there but either way keep a log measure it out get an idea of how much calcium you're getting from your food and then supplement as needed to get up to that 1200 ish range on a daily basis as well as daily impact exercises in the clinic i really stress to my clients you want to lift as heavy as you can safely and for what you may have going on so a lot of my clients that do have osteoporosis are 58 or age 50 and older and so i'll tell them look you may have some other things going on for example if you have knee arthritis you may not want to do heavy back squats because that'll just blow up your knee make you sore and then you're not going to want to go back to exercise so you want to go as heavy as you can that is safe for you higher impact activities if you're playing pickleball tennis multi-directional activities with impact are really great for building bone density walking for example isn't great for building bone density because there's not enough resistance there's not enough impact whereas walking is great exercise but ideally you really want to have higher impact by weights if you can so after one year what are the results of the study say amazingly the supplement and exercise group did just as well when they rescanned and did another bone density as the group that did the medications for osteoporosis so what that tells us is that as long as you're consistent with your supplementation and your exercises you may have another option if you don't want to be the one that takes the prescription medication the biphosphonates or the phasomax or anything like that for your bone density before you start any supplementation make sure you talk to your physician about this print out the comb study ask your physician about them make sure that this cocktail of supplements is going to be something that works for you i just thought this was a great study it's a great option that i give my clients and you know it's always nice to know that we can try to take care of things without necessarily pharmacology if we don't need it i'm not anti-medication at all but i always Unfortunately, the COMB Study combined both an exercise program and the supplements. There's no way to know if perhaps the exercises alone were responsible for the increase in BMD, though there have been other studies that look at these supplements individually.
I take Strontium and Egg Shell supplementation as well as orher supps. No added calcium as some supps encourage calcium absorption. Too much calcium not good for kidneys. Precis as could write a book on experiences. Not a candidate for "bone meds" or infusions.
" always speak to you physician" while they take a basic course on nutirtion and have no clue about herbs and natural ways.

I've been diagnosed with osteoporosis, likely from my necessary opioid treatment for a never-ending, intractable migraine that has persisted for over a decade.
While in the military I suffered multiple concussions from everything to being too close to close air support bombing runs, the same for grenade shockwaves, and taking multiple blows to my head in CQB, or as we called it hand to hand combat.
Not fully understanding the risks, after I separated from government service, I boxed and fought in early MM fights.
I have wicked osteoarthritis from parachuting and fast roping as well as running.
I do have lowered testosterone levels, though my physician has been hesitant to prescrib TRT for me.
I would definitely prefer to take on this problem without going on the current osteoporosis meds. Meds got me into this problem, I'm skeptical that meds will fix it.

I was just diagnosed with osteoporosis yesterday 4/30/25. My doctor wanted to put me on one of the meds. I really don’t want to go on medication that could have severe side effects if something more natural would work just as well. I am going to see an osteoporosis specialist and mention the “cocktail” you mentioned. I would like to try it for even a year, retest my bone density next year and hopefully my score improves or has at least kept steady. If it doesn’t, then maybe I would try the medication.
My grandmother took fosmax she passed at 78 of lymphoma and kidney failure. I take a multivitamin that has or add D3 and K 2 .

I read that taking biphosfonates prevent the body from replacing old bone with new bone, so it looks good on a DEXA scan, but you are left with old brittle bone. I refuse to take them even though my doctor wants me to for osteopenia.
i got -t3.5 on spine and 2.8 on rest what can i do i'm scared. i have low BMI.
The doctors prescribed me acid reflux pills for life, I took them for more than 10 years and now it turns out that these pills have caused the Osteoporosis I have now. Doctors now want me to take pills for life to treat osteoporosis. I was cured of my gastric reflux researching myself, and I found the 5 vitamins which I took just 6 months and I no longer had gastric reflux, now I only take them once a day and sometimes I do not take them but I feel that I am cured already but because it is natural I continue to take them now and again. Now for sure I will be able to do the same with my osteoporosis, I will do the same now I will investigate and I will be my doctor and I will cure myself with natural things without side effects.

DEXA or DXA scan can be performed and results assessed to understand if Osteopenia (T-score -1 to -2.5) or Osteoporosis (T-Score of -2.5 or less) has set in
Most patients are not even aware when Osteopenia sets in.
They must look for fatigue,weakness and knee pain as these are classic symptoms of this condition
Getting enough calcium and Vitamin D (Per RDA) and exercising is important for bone health

Some proivders caution against sports where you are twisting or bending. You mentioned tennis for good bone impact. What do you think about bending motions for those with osteporosis?
I think the biological way actually grows new bone. My understanding of the drugs is that many stop the osteoclast from cleaning out the old bone so the osteoblasts lay down over the old bone so you are building on old bone. I'm still researchig and I will check out the COMBS study. Would appreciate your thoughts. Also probably more risk with the drugs than natures way. Thank you.

Exercise and diet..my bone density improved at my 2 year scan. I'm delighted, refused to medication
I was diagnosed with osteoporosis 25 years ago. I have taken different meds at different times along with supplements and exercise. I don't think I'm able to lift heavy enough weights to make a real difference. I haven't taken any meds for 14 years but currently I'm considering Forteo again. I"m not a fan of anti-resorptives because all they do is preserve old bone.

Hi, was diagnosed with bad osteoporosis with a recent bone density scan. I took to much vitamin D as a result the results from my blood test showed my toxicity level was 5× higher then normal so had to stop taking for a while as of now can go back taking it again my doctor recommended I take between 800 to 1000 IU maximum D and 1200 calsium and prescribed medication which I refuse to take what would you recommend taking? afraid of the high calsium 1200 and not to high of dosage of vitamin D want to reverse my osteoporosis without damaging my health a lot of supplements contain vitamin D in them unaware I took to much experienced numbness and tingling in my legs and feet it's starting to subside a little my doctor wants to do another blood test to check my B1 levels don't want neuropathy issues either hope I don't have any nerve damage as a result abusing the vitamin D.

Not enough protein in the diet is as responsible for Osteoporosis as it is for Sarcopenia. Bone generation is highly dependent on protein along with stress. Yes dietary calcium is important but bone synthesis is highly dependent on protein as is muscle synthesis. Older people need more protein not less to trigger both bone and muscle synthesis.
