living with at least one of them and today we are exploring the connection between autoimmune diseases and your diet so could what you're eating actually trigger one could that be the cause or could dietary changes actually improve your symptoms is it even possible to reverse an autoimmune disorder well those are questions we're going to be tackling with Dr Neil Barnard he is an Adjunct professor of medicine at the George Washington University School of Medicine author of more than 100 scientific Publications and 20 books including his latest the power Foods diet and I'm building him up because he is in fact a leading voice in nutrition and it is a complicated topic that we have here today so if there's a question you have for Dr Barnard post it in the comments or in the chat we'll get to as many as we can when we open up the doctor's mailbag and with that we do welcome the author of the power Foods diet back to the exam room live Dr Neil Barnard thanks for being here again my friend well it's great to be with you Chuck this is a really complicated one from what I do understand about autoimmune disorders is that they are often extraordinarily complex diseases they're complicated uh but what they actually are can be explained fairly simply um autoimmunity means that auto is it's something something that's happening to your own body and Immunity is you have white blood cells they're in your bloodstream the red blood cells are carrying oxygen but the white blood cells are their security guards they are going around looking for bacteria or viruses or thing foreign proteins things that things that don't belong there and the white blood cells will shoot out antibodies you can think of them as little arrows or Torpedoes or something that they're that the white blood cell is sending out in order to attack the virus and knock it out the problem is they're supposed to be attacking viruses but sometimes those Torpedoes can attack you classic case uh rheumatoid arthritis my joints hurt you look inside inside the joint and it's red and it's inflamed and it's painful and over time it's being destroyed what's doing the damage those Torpedoes that your own white blood cells made so that's an autoimmune disorder and uh in the then the question is what triggers it and that's where Foods come in absolutely we're going to talk about that in just a second but there are as we said at the top between 80 and depending on who you ask as many as a hundred known autoimmune disorders what are some of the more common ones that we know about well we mentioned ruid arthritis that's a really common one but but as you said there are many many others and type one diabetes uh at first it was not known why did kids uh lose their ability to make insulin what was happening in the pancreas that its ability to create insulin was dying off and researchers found that the beta cells in the pancreas that make insulin they're being attacked by these Torpedoes they're being attacked by antibodies made within your own body and so that's obviously a huge one uh hypothyroidism your thyroid here at the base of your neck uh makes thyroid hormone responsible for energy if you're hypothyroid the most common cas in common region in the United States is again an autoimmune one and hyperthyroidism if your thyroid just can't turn off same story it's also uh an autoimmune condition celiac disease you okay you have gluten and it rips up your intestinal tract the gluten is triggering an autoimmune reaction there too we see it with crohn's we see it with ulcerative colitis psoriasis and and some really real surprises multiple sclerosis where we see neurological symptoms uh has turned out to be autoimmune as well those are that's just a handful and there are many more as you said CH all right so let's jump in take question right off the top Liam wondering what are the more common autoimmune diseases that are in fact affected by diet is it all of the ones that you've mentioned or is it all of them period I believe I'm going to well it's certainly all the ones that I mentioned um which is to say that when people change their diets it off they often improve um and we've been able to do a little bit better than that we've been able to identify certain foods that seem to be more culprits than others uh but I have to say I think in response to the question I suspect that you're going to see a role for food in all of them and the reason why is that when researchers have tried to figure out what is triggering this immune response it it doesn't just happen on its own it's not like the white blood cells just decided to shoot out their Torpedoes it appears that something triggered them and that something could be a protein that the right blood cell thought was a virus but in fact it was a protein that came from a dairy product piece of cheese or something like that the in fact back in 1992 I believe it was the New England Journal of Medicine uh looked at type 1 diabetes and they found that there was uh in all of these kids who had type 1 diabetes they found antibodies to a Dairy protein and so the theory was that your the dairy protein enters your body through your diet it somehow leaks into your blood the white blood cells spot it they create antibodies to try to destroy it because they think it's an Invader and those antibodies then ended up destroying the beta producing the beta cells of the panras that produce insulin right you mentioned Dairy John is wondering what other specific foods are known to trigger autoimmune reactions are there some common ones yeah and and Dair is really at the top of the list and and let me take just a minute about this because we're used to looking at issues with dairy you know dairy products dairy products are the leading source of saturated fat that's linked to heart disease linked to alzheimer's disease but I don't believe that's the issue in autoimmune conditions Dairy has sugar in it that a lot of people can't digest I don't think that's the issue either I suspect it is dairy proteins and uh there are also certain hormones in it there's there's bovine insulin which is also a protein the proteins seem to be the things that will that will trigger it okay but after after dairy uh years ago I wrote a book called foods that fight Pain and we looked at a number of autoimmune conditions like rheumatoid arthritis and others and I went through all of the articles that had picked out dietary triggers for all of them and I came up with a list of foods that I would put in order based on how frequently they were identified as being triggers the number one number two was chocolate it may not be the chocolate it might be that chocolate is often Ser mixed with dairy products uh milk chocolate uh is kind of the rule number three was eggs number four was citrus fruits and there's nothing bad about a citrus fruit normally but if you have a reaction to them then that could be an issue and then fifth was meat and it could be any kind of meat including fish including chicken any kind of meat uh the next was wheat but here I think the wheat uh wheat can cause allergies but the big issue with wheat really is that it's a gluten containing food most people no issue but about 1% or slightly less than 1% of people have celiac disease the wheat will be a trigger and so will Ryan barley okay next up nuts nuts fine for most people they're a little fatty so you got to be careful about that but most people are not allergic some are and that's one of the triggers and then at the bottom of the list are some peculiarities tomatoes onions corn apples bananas most people tolerate them really really well but these are rather rare causes of symptomology so that's my list dairy products chocolate eggs citrus fruits meat wheat nuts tomatoes onions corn apples bananas and Chuck if I got one more second I just want to tell you about what do we do with this list what I do with this list is we take them all out of the diet for a person with autoimmune conditions let them their symptoms settle down and then bring them back in one at a time every two days to see which food might be responsible it seems kind of like a an arduous process but often times these are slow to diagnose diseases and a person when they have been bounced around from rheumatologist to Primary Care to other Specialists for sometimes months and even years they're at the end of the rope and willing to try anything so a wash out type of diet like you're talking about really is not or an Elimination Diet that you're talking about is not really that big of a deal to a person at that point they're willing to just kind go for it it's really worth doing um because you only have to do it once you do it once in your life and for a person who's got rheumatoid arthritis and your joints are being deformed before your very eyes and your rheumatologist is going to put you on a biologic drug for it and they they're helpful but the cost is enormous and you think well let me just see if there is some food that I'm reacting to and it may well be one of the ones we've tried so what what you do is uh if you want to follow this um eat food FS that we know don't really cause pain but they they don't they don't trigger autoimmune conditions rice well-cooked green vegetables for example and fruits you can cook them too sort of like applesauce that kind of stuff and so you don't restrict the portions but what you're eating is a really limited number of foods and you do this for a week or two you just put up with it um and let your symptoms cool down and then put the foods back in starting at the bottom of the list so have bananas and you want to see if it's a trigger so have four or five bananas uh or maybe more than that over about a two-day period and if it doesn't trigger a problem fine go on to the next apples and for most people it doesn't trigger a problem but have two or three apples a day for a couple days and then bring in corn bring in onions you're going working back up the list um I suggest you not bring back the dairy or the meat you're better off without those things same with eggs uh but you'll you'll very often find either a trigger or more than one trigger and when you do it is the best gift because we have seen people where rheuma arthritis went away their asthma went away their migraines went away their psoriasis went away or these things improved dramatically so it is it is something you only have to do once and it's really well worth doing uh for the details you'll see them in my book foods that fight Pain I also reped it in the cheese trap in the appendix because because cheese and dairy are vict trigger and we'll go ahead and drop links to both in the show description and in the episode notes for you if you're watching this or listening to it on demand um you mentioned Tomatoes a while ago Sarah has a question about tomatoes and nightshade vegetables in particular she's wondering if those nightshades like tomatoes or Peppers uh are are really they're known very widely to worsen the symptoms of autoimmune diseases so we know Tomatoes is a big yes but what about the other night shades um I think for all of them I I think the concerns about them are somewhat exaggerated because they're not the big common triggers um but they but they can be triggers and the night shades include Tomatoes but they also include uh things like egg plant but I wouldn't reject them as a group I would test them individual individually all right let's talk about some other diets here Mia is wondering if we could compare a plant-based diet a low carb keto diet and the Mediterranean diet in terms of management for an autoimmune disorder oh vegan diet head head and shoulders above all the others why because you're eliminating the big triggers um you're not going to have any dairy you're not going to have any eggs you're not going to have any meat um so that is the place to start and for many people that's end of the story um they don't need to go any further The Elimination Diet that I described is something you can add but no a ketogenic diet it often leaves in some of the worst triggers that that a person could consume and same with Mediterranean unfortunately and of those three diets uh the plant-based diet the vegan diet by far the most anti-inflammatory by my understanding David is wondering if you could elaborate a little bit more on the connection between inflammation and the body and how that might relate to autoimmune disorders okay when you when the antigen the protein has entered your body and your body is responding to it it's creating antibodies to try and knock it out that's when the inflammatory process gets into high gear um because how these antibodies often work is by creating an inflammatory process inflammation is is one of the tools your body uses to destroy uh Invaders the problem is that the inflammation ends up destroying you or hurting your joints or causing damage of other kind so the two are intimately related and they're kind of parts of the same coin yeah it reminds me of that book uh that Dr Monica agarwall our friend has written called body on fire all about her own experience with it and and how the body responds to certain triggers and and to hear a term like it felt like my body was on fire that paints quite the picture Dr barard not not a pleasant one it's exactly how it feels I mean if when you go to the store and you see people in the line buying ibuprofen they're buying Advil and so forth for for inflammatory pain that's what their body feels like that's what they're dealing with so know it's it's a it's a real thing and uh the flip side of it is when you find the answer to it if if you have found the cause of it and you can prevent the flareups from ever happening oh that's like golden all right and we know uh speaking of uh things like ibuprofen or pill bottles uh let's talk a little bit about supplements here we know that a plant-based diet a vegan diet we're told to supplement with B12 well Rachel is wondering whether there are any dietary supplements that can benefit somebody who's suffering from an autoimmune disorder there was a Harvard study that came out in 2022 where they put this to the test and they looked at Omega-3s and they found that they really weren't very particularly helpful um but they looked at vitamin D and it was helpful it was it wasn't huge but it led to oh 20 22% reduction in the incidence of autoimmune conditions so we'll take it um vitamin D is something that normally comes from the Sun but a lot of people are indoors or using sunscreen so you don't get the natural source so a lot of doctors would say about 2,000 international units is the amount that they want you to have and that's that's a sensible recommendation all right so before you can check out with the vitamin D before you can check out with the ibuprofen you have to get to the checkout register itself and right next to that is a big old row of candy top to bottom usually four or five shelves John is wondering if you can uh you know not resist this temptation how does sugar consumption impact inflammation and autoimmune diseases there has been a lot of suggestion that Sugar might make them worse the mechanism has never been entirely clear and many of us have wondered if it's the fact of the matter is that sugar is often packed into people don't people don't e just a box of Domino Sugar it's always packed in with something else and so that sugary candy bark what's what's in it Dairy chocolate two of the big triggers uh so U sugar doesn't seem to be helpful it does seem to be a problem the mechis not clear but but it could be the company that it keeps all right uh Emily is wondering whether there's any benefit to including fermented food in the diet if you have an autoimmune disorder fermented foods do have benefits um but I'm not aware of them actually making a big difference for autoimmune autoimmune conditions let's talk fiber something that's found in abundance in a plant-based diet Michael can a high fiber diet help improve the symptoms for someone with an autoimmune disorder where fiber really comes in in a big way is with the ones in the gut we're talking about Crohn's disease we're talking about aler of colitis in particular um and high-fiber foods have two advantages one is what they are not um there is no fiber in meat so a high-fiber diet means the meat is gone that does a huge benefit there's no no fiber in Dairy getting rid of dairy is super uh anybody who's got had Crohn's disease or aler of colitis will tell you they do a lot better by avoiding those products but the other thing is that fiber comes from Beans comes from whole grains comes from vegetables comes from fruit it comes it goes through your body and it absorbs water and it's one of the most gentle things for your digestive tract so for people who do have the gut rated autoimmune conditions high-fiber foods are really going to be helpful to you now look at different kinds of fiber you might find that you do better with some than others but but you don't want to miss your fiber all right uh speaking of fiber you can find Fiber and fruit David is also wondering whether there are any specific fruits that have been shown to benefit people trying to manage autoimmune disorders I my best guess on this is that the the fruits that you tolerate well are going to always be helpful for you because they're good they're nutritious they're vitamin rich and they've got antioxidants in them but as I mentioned earlier there are some fruits that do bother some people not it's not common but you do see once in a while apples or bananas affecting people and Chuck kind of for extra credit I have often speculated why is it that certain foods might be triggered and my anthropological answer is that human beings began as a species in Eastern Africa and from there we sort of migrated throughout the rest of the world and it wasn't until humans came over the bearing straight and ended up in north and Central and South America that we first were exposed to tomatoes to Chocolate to a number of foods and a lot of the foods that are triggers seem to be foods that weren't part of our early experien on this planet so there you have it that's probably an explanation for why some of these foods are not so well tolerated uh let's talk about tolerating red meat here Sophia is wondering whether there's a link between red meat in particular and the development of autoimmune disorders do we know if steaks hamburgers the like can actually cause the body to develop such a disorder um the cause is probably a mixture of two things uh there may well be some genetic pred disposition to it in other words your DNA May hold the ability that an autoimmune disorder could happen for you we've seen this with type 1 diabetes for example um that it tends to sort of run through families a little bit however these these genes are by no means decisive because you'll see identical twins one's got the disease the other doesn't have the disease at all and never gets it and despite the fact they have identical genes but the genes seem to be sort of permissive and then the environmental exposure the food exposure is what seems to make it actually come out now red meat Yes red meat is one of the foods that triggers it partly it's an inflammatory food but the proteins in meat may actually be a trigger too we see this both for the digestive uh conditions that we that we mentioned like ulcer of colitis but the other ones as well seem to seem to be related to meat consumption and when people avoid meat their flare ups seem to be dramatically reduced we' we've heard about people with multiple sclerosis for example uh which is a frightening condition where your body is just not working right uh sensory issues motor issues both are are are affected and when people get away from animal products completely in many cases they do dramatically better all right Tarter from Olivia how does stress relate to autoimmune diseases and can a dietary Improvement mitigate stress oh okay wow what a great question uh first of all stress causes a hormonal reaction hormonal reactions often cause your immune system to react and to not be as well regulated as it would otherwise be so I I would like to say stress is good for something but it's really not uh stress causes accelerates the the risk of all of these things uh but the other thing that stress does is when you are under stress it's April 14th and you are an accountant okay uh you know what I mean um when people are under stress what happens their diet goes to heck I mean that you're you're not you're no longer excluding the foods that are bad for you uh you might be smoking or whatever it is so stress has direct hormonal responses in the body plus it kind of unhinges our control over dietary exposures to a degree and that can cause a problem too now this the flip side let's say a person changes their diet they're on a low-fat healthy vegan diet what you do see is a lot of mood improvements and we've seen this in a number of our studies and other researchers have as well that people's moods tend to improve less anxiety less depression but also sometimes less of a stress response and better more restorative sleep so diet isn't the only part of the answer to these things but it is part of it all right and William is wondering whether fasting or intermittent fasting can be beneficial for someone's uh suffering ing from an autoimmune disorder Dr barard yeah it sure can um fasting sort of lets the the body rest um and with many many autoimmune conditions you see fasting improving them now that said fasting can be a dangerous thing so don't just decide okay I'm gonna fast for a month and I'm gonna do it at home and see what happens um you need to be supervised for any kind of prolonged say a water fast so there are places like True North which is a terrific probably the best known fasting facility where you will be seen every single day by a doctor who monitors how your condition is doing making sure that you're adequately at that you're in good shape and that you're not getting into any trouble so don't try this on your own uh do do if you're going to do a serious fast get medical supervision all right let's talk specifically about the plant-based diet here again and autoimmune Windom are that's a cool name Windom are there any challenges or considerations for individuals with an autoimmune disease in following a strict plant-based diet I think if you have an autoimmune disease getting away from animal products is always a good choice but it may be that not everything that's a plant is something that you want to have I mean there are people who will have reactions to certain foods gluten products are all plant-based but some people can't have them it's less than 1% but but celiac disease is real so going plant-base is good but there are still some other Foods you're going to want to avoid all right let's uh wrap things up here we talked a little bit about nuts at the top uh Leah is wondering are there any other specific nuts or seeds that should be included or avoided for someone who is struggling with one of these disorders you want to do a test um and your doctor could test you for allergies there are certain tests that allergists do fine idea but you can also test yourself um if you want to to see if there are other reactions you may have to them so avoid all nuts and you can put them back into your diet bit by bit uh the main uh distinction that people often make is that peanuts are actually actually not related to the other nuts they are a legume they are more closely related to a soybean or to a black bean than they are to a walnut but um many people do react to peanuts that's back to my new world food um that's something that people did not have in in rural Africa as we as our body was uh developing through the process of evolution so give give it a test and see what you find all right final one comes to us from Benjamin are there any specific nutrients or Foods that can help support the immune system of someone who's struggling with an autoimmune disorder okay you want an overall healthy diet um you want four food groups those are fruits you want vegetables you want whole grains and the Bean Group or the legume group you want to have all of those and that's a good overall healthy diet you do want to take vitamin B12 because you need that for healthy nerves and healthy blood and then some people will go further and probably the number one thing that I hear people talk about in addition to vitamin D which I mentioned earlier is turmeric uh turmeric has been used for countless years um as an anti-inflammatory and so much so that you will see many many commercial brands of turmeric you can also buy the spice itself it is in fact an anti-inflammatory and you can put that to work too all right and you talk about turmeric and a whole lot more I'm sure in the power Foods diet your your newest book that is available for purchase on Amazon and your local book retailers right now there's a would imagine a lot of some of the principles we were talking about today kind of may have found their way organically into the pages of the book as well they do the power Foods diet really focuses on My Quick note on the Foods that can trigger autoimmune response: 1) dairy,2) chocolate ( the milk in milk chocolate) 3) eggs, 4) citrus fruits if you have a reaction to Them, 5) any kind of Meat ( chicken, fish, red Meat, pig) , 5) gluten in wheat or Rye or barley ( 1% of people are gluten intolerant but most are not) 7) tomatoes, 8) nuts and for some people 9) onions, corn, Apples, bananas, but most people tolerante Them really Well) – and if you are 100% plant based remember to take B12 supplement and all seemed to could benefit from 2000 international units of D vit if they do not get enough sun 🙏🍀💚🐸🍀

A lot of people have achieved remission from a carnivore diet. My sister is trying it for her psoriatic arthritis. Makes sense since plants have anti nutrients.

Thank you Dr Barnard for the valuable information. I was diagnosed with RA some years ago. I am taking ayurvedic medicine + gradually my diet becoming plant based. I was becoming much better but some weeks back i had increasing lots of pain that i could not sleep and it was affecting my work. But with prayers and using hot and cold water therapy my pain is gone. Barbara O Neill 's advise on water therapy really helps. And now i know that i have to continue taking the hydro therapy till the auto immune is reversed along with medication, plant based diet ,avoiding certain plant food that triggers,exercise,good sleep, sunshine and reducing stress with trust in divine providence .the reason why im writing this is because only those who suffer from RA will know how unbearable the pain is . Even taking a hot shower for 3 minutes and switching to cold shower for 30 seconds,repeating 3 times takes away the pain.

I have founded with my own "patients", it was ANIMAL MEAT. Few Lupus patient avoid attacks by not consuming beef. But Lori was a real tragic case for me. She was doing well for 2 years, not one attack. Then one day her NEW BOYFRIEND gave her a PIECE OF PORK, and she ate it, Lori has what called a CYTOKINE STORM ( you heard with people with COVID) today, her immune attacked her Gallbladder relentlessly and her gallbladder had to be surgical REMOVED !!!. I lost her, she never was well again for 2 years, finally took off to Thailand she say, to see if Thailand have effective protocol.
So the vitamin D receptors are everywhere. Seems to me that they are the switches and vitamin D comes along and turns them on/off. Low vitamin D and a device is either left on or not turned off efficiently. However it only works once, so you need sun every day.

Mass produced food, gluten and pepper in my case. I am vegan so all non vegan food is exluded from my diet. My arthritis is connected to psoriasis. I thought i should drop tomatoes but i have a beautiful tomatoes in my garden and ate them despite of warnings and am just fine. Maybe my previous reaction to tomatoes was to sprayed, bought tomato.
