foreign [Music] what's up guys Dr Alex Tatum here today we're going to talk about some of the lifestyle recommendations that we make to men who are trying to conceive in addition to discussing some of the medical treatments for male fertility this is our second entry in our four-part series if you haven't checked out our first video where we discuss what the initial workup for male infertility lifestyle these factors are things that everybody should try to optimize as they seek to improve their fertility so if you're a man trying to achieve pregnancy listen up because we're going to discuss some of the positive changes that you can make to improve your fertility right now no doctor's visit required the science on this is very clear obesity poor diet and lack of exercise can be devastating to a man's sperm counts for many men their fertility is the canary in the coal mine when it comes to their overall health you can live a pretty unhealthy Life as a young man and while those choices may not cause you to have a heart attack in your 20s or 30s your fertility may already be paying the price so if you want to maximize your fertility improve your lifestyle when it comes to diet refined sugars and processed carbs are your enemy if it comes in a box or can you probably shouldn't be eating it food is meant to have an expiration date so make things easy I try to tell my patients to eat around the edges of the grocery store all the fresh meat fruits and veggies are generally sold here so if you find yourself straying into the middle aisles think twice to make sure you're making good choices this advice extends to all of your meals lunch and snacks included that may mean for a lot of working guys starting to pack fresh food from home rather than relying on fast food while on the job now prescribing a certain exercise regimen is beyond the scope of this video but exercise itself is essential for both fertility and overall health I recommend doing something to raise your heart rate and keep it up for at least 30 minutes a day every day cardio like running or the stationary bike is a great option but if possible should be combined with some high intensity resistance training if you're burning fat and building muscle you're helping to improve your health overall which in turn can help your fertility something else that men can change to improve their fertility is to stop all tobacco and nicotine usage it has been proven beyond a shadow of a doubt that both smoking and vaping can significantly decrease both sperm counts and motility regardless about otherwise healthy that person is they can also increase a negative risk factor known as sperm DNA fragmentation which when elevated can decrease a man's chance of conception and increase the risk of miscarriage these sorts of negative effects on fertility have been shown with almost all recreational drugs this includes alcohol marijuana and narcotic pain medication among other substances no amount is considered safe or okay when it comes to fertility as a result I recommend that my patients try to completely cut out all recreational substances when trying to conceive or at least reduce their use as much as possible yes I know this is a literal buzz kill but the data is pretty clear on this one now caffeine is a little more of a gray area many Studies have been published looking at the effect of caffeine on male fertility but the results are inconclusive generally speaking caffeine intake hasn't shown a big impact on sperm counts at least in most studies but it has been associated with potential changes in sperm DNA fragmentation and in some studies was correlated with a delayed time until conception so I typically recommend that patients moderate their caffeine intake a small cup of coffee or two each day is probably fine multiple energy drinks or entire pots of coffee probably aren't so what about vitamins and supplements this is an area of significant debate amongst experts some Studies have shown a positive benefit with certain vitamins while other studies looking at the exact same supplements haven't shown any helpful change at all since there's no definitive answer here my recommendation has always been that if a patient wants to take supplements in addition to making healthy lifestyle changes I would go with just two those are irregular daily men's multivitamin and coenzyme q-10 also known as CoQ10 a daily men's multivitamin contains most of the vitamins and minerals that have ever been shown to even have a potentially positive effect on male fertility and CoQ10 has shown unique promise in specifically improving sperm motility now these can be purchased at any big box store like Walmart or Costco so let's say a guy makes all these positive changes he starts exercising eating right taking his vitamins and he cuts out all recreational substances when can he expect to see results in his fertility broadly speaking about 74 days or just shy of three months this is because it takes the male body an average of 74 days to make new sperm from start to finish so almost any positive change we make when treating a man's fertility will take at least least three months to fully take effect this is true for any lifestyle change medication or procedure this is important to remember as we move on to discuss some of our Medical Treatments for male fertility so now let's talk about the role of medications and male fertility treatment most of the medications that we use in male fertility treatment are designed to alter certain hormone levels hormones are chemical signals that different parts of the body use to communicate with each other via the bloodstream and proper sperm production demands that certain hormones be imbalance it's all part of a complex system of checks and balances that must be aligned in order for things to work properly that's why we always check hormone levels when evaluating a man's fertility these hormone levels can be altered using different medications think of a scale if you can imagine a scale that is out of balance you can also probably see how pushing on one end of that scale with just the right amount of force could make things equal again that's essentially what we try to do with medications in the world of male fertility there is a lot of debate in this Arena regarding which medications are the best to use and which patients are the best candidates to use them in some of the most commonly prescribed medications include clomiphene citrate also known as Clomid a nastrozole also known as Arimidex human chorionic gonadotropin also known as HCG and follicle stimulating hormone also known as FSH in reality there are even more than just these and talking about the differences between each of them and when they should or shouldn't be used is beyond the scope of this video but it's important to mention that they do exist and are frequently used when appropriate when a patient is started on hormonal therapy as part of their fertility treatment it's important to monitor both hormone levels and semen analysis parameters in our practice this usually means visits every few weeks or months to evaluate a patient's progress this kind of hormonal therapy is commonly combined with other treatments as well including the previously mentioned lifestyle changes and not uncommonly some fertility procedures which we'll discuss in our next videos I hope you found this video educational remember if you're struggling to conceive the lifestyle improvements
