welcome back guys today i have a very important but somewhat confusing and slightly overwhelming topic and that is your skincare routine i mean this is such an important topic and a question i get over and over again about you know what ingredients should i be using should i be mixing when should i be using these how often and so today i'm going to offer some general advice as well as my own personal tips on how you should build your skincare routine and help you achieve your skincare goals i like to view skincare in a pyramid where at the base is the essentials of skin care and this is truly what everybody should be doing regardless of your age your skin care goals and what you're trying to achieve and then the middle tier are the products and ingredients that you want to incorporate that will help you target and achieve your particular skincare goals whether that's anti-aging you know fine lines and wrinkles or acne or hyperpigmentation hydration what have you and then the very top layer of your triangle are things that are really nice to have but there are not necessary so these may be ingredients or products that are really expensive that you may only want to use intermittently or products that should only be used intermittently because of the way they're structured or things that just makes you feel better because part of skincare is also self-care so let's start off with the foundation what are the essentials of skin care first and foremost we need to view our skin as a functioning organ just like we exercise to keep our heart healthy we need to practice daily habits in keeping our skin healthy how we keep our skin healthy and functional and intact is by cleansing moisturizing and photo protecting these three key steps are the essence of skin care because only when you have healthy functional skin will you be able to achieve all your other skincare goals trying to treat acne and trying to tackle hyperpigmentation or fine lines without doing the basics will get you nowhere and often cause more irritation and exacerbation of what you're trying to achieve now as far as how often to be doing these things it really again depends on your skin type number one the cleansing at the very least cleanse once a day and the most important cleanse is in the evening or before you sleep to thoroughly remove all the oil dirt bacteria makeup sunscreen off of your face so that way number one your skin is able to recover itself overnight and also help you better absorb the additional products to put onto your skin anytime you cleanse you should always moisturize cleansing without moisturizing can lead to further dehydrated and dry skin so moisturizing should follow cleansing now in the morning for some individuals who only just wash their face with water and definitely you still should use a moisturizer anytime that your skin is wet from water or cleansing you need to follow with a moisturizer and lastly sunscreen that should be used during the day throughout the day we recommend reapplying even every two hours if you're active out in the sun because we know that when it comes to skin damage and skin aging environmental aggressors in particular uv radiation is the skin biggest enemy so that should definitely be incorporated into your daily routine so that's the foundation and if that's where you want to keep your skincare then great for a lot of my patients who don't have a lot of skin concerns this is really all you need but for those individuals who are looking for something additional a little extra when it comes to the middle tier especially for anti-aging the two biggest ingredients that you should have in your routine are topical retinoids and alpha hydroxy acid and mainly here glycolic acid we know that topical retinoids help to stimulate collagen improve skin dullness and skin texture alpha hydroxy acids helps with gentle exfoliation but also has been shown over time to stimulate collagen and skin's natural hyaluronic acid production they've been studied is that the two actually work really well synergistically now even though they work well together we don't recommend using them at the same time because the two together can be extremely irritating unless you're able to find products and there are definitely a handful out there on the market that have the two formulated together usually in those situations the products have been tested and they're better tolerated i do not recommend layering retinoids and alpha hydroxy acid in the same routine so how may you incorporate these two in your routine using retinoids on a regular basis if you're able to maybe four to five nights out of the week and then incorporating a glycolic acid based treatment or product into your routine the other two to three nights one of my favorite ways of using alpha hydroxy acid is as a levon treatment i find that you are going to get way more benefits with levon products than you are with exfoliants that are washed off like for example in the form of a mask those mass type of alpha hydroxy acids that are removed after you know 10 to 15 minutes they only act as a superficial exfoliant they don't really have the time the contact time with your skin to be able to make those changes with your skin over time wash off masks are great if you're just trying to remove some dead skin which also is an important step when you're trying to achieve a more youthful radiant skin but to help with the fine lines and stimulation of hyaluronic acid the leave on treatments are really what's most effective so i recommend alternate on a nightly basis or split them up you know based on your days of the week or even between morning and evening depending on how well your skin tolerates these ingredients so we've reached then the top part of our pyramid and these are additional things if you choose to add them to routine may offer additional benefits and products when it comes to anti-aging includes peptide treatments peptide serums eye creams growth factors serums growth factor and peptides do have some good pulmonary study in helping stimulate collagen and improving the appearance of fine lines or wrinkles and so again whether it's neck cream eye cream peptides growth factors these are kind of really the icing on the cake that will take your anti-aging regimen up to the next level but again these are not going to be my first year of active ingredients that i will focus on just because the studies aren't quite there yet and a lot of the times it can be very much dependent on the formulation and the product but we do know that retinoids and alpha hydroxy acids have a long-standing history and have really proven themselves to be very effective in the two key ingredients that i do recommend when it comes to anti-aging so if you don't really know where to start i would definitely start off with these two ingredients now a very similar approach can be taken when it comes to treating acne that basics of skin care is still very important because we know that in acne prone skin your skin barrier is already somewhat unhealthy because of the inflammation from acne so if you just jump right into acne treatment or use too many acne treatments without moisturizing and properly cleansing it actually will worsen acne and many clinical studies have shown that so once you have your cleanser your moisturizer and sunscreen in place this is where you i recommend adding in a topical retinoid and here i recommend a dappling or prescription retinoid and then potentially like a benzoyl peroxide product and it can either be a cleanser so you can choose to substitute one of your cleansers for a medicated cleanser or it can be a benzoyl peroxide leave on treatment if you are going to be using levon treatment for benzoyl peroxide just make sure that you are separating it out from your prescription retinoid because it may deactivate the prescription retinoid it is okay to use together in combination with the dappling and then the top of your triangle could be ingredients that may help you with other concerns with your acne whether that's hyperpigmentation more congested pores and so this is where you can maybe incorporate a little salicylic acid treatment that you can use on targeted areas maybe surround your nose your chin a few times a week or say a clay mask to help with excess sebum once or twice a month now the other thing is hyperpigmentation when it comes to hyperpigmentation it's even more important that you are diligent with sunscreen and even more important that you are super diligent with your skin barrier health the reason why is that hyperpigmentation stems from inflammation and when your skin is irritated and dry dry skin can often trigger inflammation often feeds to that hyperpigmentation and furthermore a lot of the brightening products can be irritating so you must establish a foundation of a gentle cleanser moisturizer and sunscreen and then add in the other active ingredients the approach can be very similar to actually treating fine lines and wrinkles retinoids and alpha hydroxy acids that help to fine lines and wrinkles have also been very well studied in the context for treating hyperpigmentation and uneven skin tone and then the top of the triangle is where i think you can get pretty creative there are a lot of ingredients out there that can be helpful and give you additional boost in improving your uneven skin tone and these ingredients include niacinamide azolic acid transamic acid kojic acid or butane many times a lot of the products out there are formulated with a lot of these ingredients together so you know pick one that you want to use and just add that into your routine whether using in the morning or night as long as you are really mindful of how you incorporate these products to minimize irritation as far as like what to use like serum versus moisturizer it really goes back to your skin type for example you can certainly substitute your moisturizer nighttime moisturizer with retinol and that can be your moisturizer if you have super sensitive skin conversely you can also use a retinol serum as your middle tier as that treatment and then follow that with a moisturizer it just really depends on how you want to design your routine the ingredients that i mentioned are the ones that you want to look for but how to incorporate them again is very personal the other two things i want to mention is that number one whenever you're starting a skincare routine you always want to minimize the number of products you're using for various reasons number one if you're using so many products chances are quite a few of them are not even penetrating into your skin to make a true impact on your skin the benefits you're seeing are merely from just your skin the stratum corneum the top layer of your skin being hydrated and when your skin is hydrated it's going to look healthy and radiant so minimize the number of products i would say per routine do not use more than two actives so meaning you don't want to use more than two serums and if you're using an essence then skip out on the other serum and just use essence and serum limit the number of products that you use per routine also whenever you are starting a new product if you have sensitive skin make sure to test on a certain area of your body whether it's your an arm or along your jawline for a few days to make sure that you don't react to the product and three always make sure to give it enough time i would say with any product give it at least two to three months of consistent use you know when we're talking about these skin changes it's happening at a cellular level dna changes proteins that are being synthesized and that doesn't happen overnight it takes many many months for that to happen and so to translate microscopic changes to visible changes that we see on with our naked eyes i mean realistically six months or longer is not an inappropriate amount of time so definitely make sure that you commit the amount of time before you change out your routine i personally love trying various different products but when i do i make sure to just change one at a time and give it ample time to see how it works on my skin it's okay to experiment i encourage you try new products because that's really how you will be able to find products to work well for your skin but when you do make sure that you don't change them all at once and give it enough time for it to work but as long as you stay within the overall pyramid triangle kind of that guidance then you can really have fun with skincare and hopefully achieve your skincare goals What if i use a cleanser for dry skin? Should i still moisturize? And how do i know if i have dry or oily skin, i think i have neither my skin isn’t oily and isn’t dry
Love your videos..i am 32 and I've recently started take care of my skin and understand the importance of a proper skincare routine but I am finding it very hard to find a sunscreen for my oily skin. I live in a very rural area in Canada and find it very difficult to find some of the sunscreens that alot of dermatologists are recommending on here. Could you recommend a sunscreen for oily skin on a budget? THANKS

It would be quite helpful with there being so many products out there, that you actually recommend brand names of products with the ingredients that you are speaking of to make sure they have enough of them, and that they actually work. I noticed that with each section of your pyramid, you did not do that and honestly the majority of people watching your videos need name brands that actually work. Basically everyone ones healthy skin less wrinkles, etc. so throat products for skin care out there one for dry skin one for oily skin one for normal skin, etc. because wow your video was quite interesting. It just did not contain any particular information on how to pick out good products. I’ve watched a lot of reviews on different products that I have used and you find reviews on each that say they are no good and others that show false results so again specific product names for different types of skin to build a regimen would be quite helpful and I have yet to see anyone do this, but I would think as you are a doctor that you should be one of the first people to put one out there it would Be completely helpful and then everyone could get on with their skin care regimen instead of playing guessing games all the time I hope you take this into consideration. Thank you.
